To be eligible for the OAPP:

  1. Be resident of the state of Illinois

  2. have been prescribed an opioid or have a health condition or disease for which you could be prescribed an opioid

  3. be at least 21 years old or older

  4. do not hold a CDL-driver’s license or a school bus driver permit

How to register for the OAPP:


STEP ONE

OAPP Physician Certification

All applicants must have their physician fill out the physician CERtification (PC) to certify you have been prescribed an opioid. this must be submitted first in order for you to complete your registration. this require an in-person visit to your physician.

once submitted, the PC is valid for 30 days. If you wish to continue in the oapp, your physician must submit a new pc within 30 days of expiration.

STEP TWO

Create an OAPP Account

After submitting the PC, you must sign-up for an account and complete the patient registration on the illinois cannabis tracking system portal.You may not be a part of the medical cannabis pilot program and the OAPP at the same time.

you can complete this process at home or visit a licensed dispensary or local health department for assistance.

STEP THREE

Requirements for Registration

When completing your online registration, you will be asked for the following:

  1. a clear copy of your illinois driver’s license or state id.

  2. proof of address documentation such as a PAY STUB, BANK STATEMENT, UTILITY BILL, ETC.

  3. A passport-style Photograph MUST BE SUBMITTED. the use of a pASSPORT PHOTO VENDOR, such as walgreens, is RECOMMENDED. IF you already have a photo, it can be resized using the photo tool provided by the state department.

  4. co-payment of $10 that will be submitted via state epay system.

  5. designated dispensary that you will be able to purchase from.


STEP FOUR

Visit a Dispensary When Ready to Buy!

you will receive an email with your registry card information once you finish your REGISTRATION. take this information to a dispensary when you decide you are ready to buy products. be sure to bring an id and your oapp registry information every time you visit!


STEP FIVE

Re-Enrollment

if you decide to re-enroll in the oapp program, you must contact your PHYSICIAN for a new APPOINTMENT to review your continued participation in the program. At this visit they must submit a new pc within 30 days of your registration EXPIRING. Once submitted, you must update your registration and pay the $10 co-payment again to receive an updated registry card.
